A GOOD SAMARITAN:
A good samaritan is described as "one ready and generous in helping those in distress". This is what the INFOCARD is all about - providing information to a good samaritan so that they can knowledgeably and immediately provide effective assistance to a person in need.

DESCRIPTION:
If you are injured or have a serious health problem in a place where no-one knows you, one can only hope that a good samaritan will come to your aid or, at the very least, call for help. A person from out of town may not know the emergency number or may not be able to think clearly at the time. The INFOCARD draws the good samiritan's attention to the emergency numbers. If you have a defined medical condition, it will be helpful for the good samaritan to know what to do and what not to do immediately and what information to give to the person on the other end of the emergency call. Depending on the circumstances, the good samaritan may decide to call a relative or close friend of yours, or to bundle you into a taxi telling the driver your address. The INFOCARD provides a wealth of information to best help you in your time of need. If the good samaritan believes that an emergency call is necessary, the INFOCARD will provide the emergency response team with immediate information pertaining to any specific medical condition you may have, any drug allergies, medication currently used, blood type and the name and phone number of your doctor. The photograph confirms that the information relates to you. As every individual has different needs, the back of the INFOCARD can be used to display more detailed medical information (ask your doctor to help) or just a list of phone numbers.

USAGE EXAMPLES:
1. If you have pills to contain a heart condition, it would be helpful for the good samaritan to know how to identify and administer them.
2. If you are having an epileptic seizure, the good samaritan will need to know how to make you comfortable without hurting you or getting hurt themselves.

INFOCARD PLACEMENT:
To ensure that you have your INFOCARD with you everytime you go out, it is best to place it in something that you are not likely to forget, such as a wallet or purse. It can also be worn around the neck or clipped to a pocket or belt. However, it is one thing to carry an INFOCARD with you and another to let a good samaritan know that you have one if it is not immediately visible. A suitably etched bracelet, neck chain or brooch can perform this function.

The price for a single INFOCARD is $19.95. If you would like an instant digital photograph taken, the price will be $9.95 extra. Ask about discounted rates for families of three or more.

This will give you time to complete the form, contact your doctor if necessary and choose a suitable photograph (unless you would like me to take an instant digital one when I complete the order). Once this has been done, contact me by email or phone to make an appointment. I will come to your home, fill the order per the completed order form, either scan an existing photo or take a new one (extra cost), and leave you with the finished laminated INFOCARD.
